If your child is approved for Habilitation or Attendant Care services through DDD/ALTCS, you may be eligible to be one of your child's paid providers for these services. If you have questions, contact our Family Support Team.

PAY RATES
AZA United currently pays Parent Providers the following rates:

$21/hour for Habilitation services
$19/hour for Attendant Care services

This application is ONLY for PARENTS/GUARDIANS who would like to work with their own children (regardless of the child's age).

Please note that parents may provide Habilitation and Attendant Care services only (not Respite). You can also have additional providers that work with your child for any of these services. Contact your DDD support coordinator if you need to request an increase in the number of hours your child is currently authorized to receive. Why AZA United?
Arizona Autism United is a leading nonprofit organization serving children and families affected by autism in our community. We have been a leading agency of choice for many Parent Providers across the state of Arizona since the program began in 2020. Our Habilitation Field Support Team can do videocalls with you to provide help with behavioral strategies, teaching techniques for your child, structuring your child's day, creating visual supports, and so on.

In addition to highly competitive pay rates, we have streamlined our systems to make sure that the process of getting started as fast and easy as possible for parents in this role. Parent Providers are independent contractors, which allows for flexibility as your child's needs change over time.

Parent Providers for AZA United are classified as independent contractors, and not employees. Please confirm that you are willing to move forward with this process as an independent contractor.*

This role requires you to be at least 18 years old. Are you at least 18 years of age?*

This position requires an Arizona Department of Public Safety Level One Fingerprint Clearance Card. If you do not currently have one, AZA United can provide you with information on how to apply. If you do not already have a Level One Fingerprint Clearance Card, are you willing to apply for one?*

This position requires the following certifications: CPR, First Aid, Article 9. It is the applicant's responsibility to obtain these certifications. AZA United will provide options and may offer some classes for you to complete these certifications. Are you willing to obtain these certifications if you do not already have them?*

This position requires a personal email account and basic computer skills to submit online time sheets and data reports. You may use a personal or public computer. Are you willing and able to meet these requirements?*

AZA United requires, that following hire, all payment for time worked (e.g., your paycheck) is made to our employees and/or contractors via direct deposit OR that all physical paychecks are physically picked up from our main office location in Phoenix. We do NOT mail paychecks. Is this acceptable to you?*
