Parent Provider Application - TUCSON AREA
The Arizona Division of Developmental Disabilities (DDD) has approved parents to be direct care providers for their children under age 18. This program has been extended until 2024.

AZA United is compensating parent providers in Southern Arizona (all areas south of Marana) at the following rates:
- Habilitation = $22.00/hour
- Attendant Care = $20.00/hour
Next Steps for Interested Parents
AZA United is committed to doing all we can to serve and support families, and we are pleased to be able to offer this opportunity for interested parents. The first step is to fill out this application and then we will contact you with next steps.
This application is ONLY for parents/guardians who would like to work with their own children. All other applicants (siblings, family members, referrals, parents of adult children, etc.) should still use our standard Habilitation & Respite job application.
Please note that parents may provide Habilitation and Attendant Care services only (not Respite). You can also have additional providers that work with your child for any of these services. Contact your DDD support coordinator if you need to request an increase in the number of hours your child is currently authorized to receive. Temporary Direct Care Workers will be independent contractors, which allows for greater flexibility and higher pay than our standard Habilitation & Respite Provider position.
